NATURE OF PRACTICE: Lightbulb Radiology, by and through its physician(s), provides second opinion imaging services with face-to-face audio-video consults with the purpose of reviewing your results with you. All services offered on the Website are collectively referred to herein as the “Services.”
LIMITATIONS: The Services do not replace a full history, physical exam, and medical evaluation by your physician. By using this site, you agree that you will review these second opinion findings with your physician in order to establish diagnoses and guide treatment. You understand that Lightbulb Radiology DOES NOT: (1) diagnose or treat medical conditions; (2) perform histories, physical exams, or perform routine medical evaluations; (3) perform, order or refer individuals for diagnostic imaging services, order or refer individuals for prescriptions or provide any other medical services; (4) provide medical services beyond the explanation and interpretation of radiology studies for which you have already received results; (5) provide explanations or interpretations, or serve as expert witnesses for purposes of any litigation; (6) review the records of minors; or (7) handle emergencies.
Lightbulb Radiology will not be able to correlate your imaging findings with a physical exam and will not be able to learn information that would be garnered during an in person examination. Lightbulb Radiology will not be aware of pieces of medical history and other signs and symptoms to the same extent as your physician. For these reasons, any opinion generated during this process should be considered limited and not to be acted upon without correlation by your physician.
USER OBLIGATIONS: The Services may provide you with additional information that may assist you and your physician in making decisions about your medical care. In using the Website, you agree to the following:
That you will, at all times, provide true, accurate, current, authorized, and complete information when submitting information or materials to the Website. If any false, inaccurate, untrue, unauthorized or incomplete information is submitted by you, Lightbulb Radiology reserves the right to terminate your access and use of the Website.
That you have a physician licensed in the state in which you reside who is responsible for your care, and who will remain responsible for your care including, but not limited to, any and all aspects of diagnosis, prognosis, treatment, and follow-up.
The Services do not replace an in-person visit with a physician and that the Services will not be used as a basis to refuse or delay medical treatment from your physician.
That you are fully responsible for conveying the information provided to you by Lightbulb Radiology to your physician.
That you will not use the Services in any legal proceeding without the express written permission of Lightbulb Radiology.

This list is subject to addition and change at any time. Lightbulb Radiology is not responsible for the actions or inactions of such third parties, and makes no guarantees regarding their services. If you have complaints, claims or questions regarding such third parties, please direct them to the appropriate third party.
PAYMENT AND CREDIT TRANSACTIONS: You are solely responsible for all fees and expenses charged by Lightbulb Radiology (see pricing section) and understand that you must pay these fees in advance of the interpretation and video consult. You have the option to withdraw consent for your second opinion at any time but no refunds will be administered after the payment is made. You understand and agree that the Services are not covered by insurance, and Lightbulb Radiology does not contract with any health insurance company or payor for the Services. Information submitted on the Website is transmitted to third parties as necessary for payment processing.
